Neuroplasticity-Driven Stroke Rehabilitation: Validating Motor Imagery with fNIRS and Aphasia Evaluation via Functional Connectivity Analysis

Stroke remains a leading cause of long-term disability. Our research team is dedicated to developing and validating advanced multimodal technologies to achieve precision assessment and efficacious rehabilitation of both motor and cognitive impairments following stroke, with a particular focus on investigating into advanced, mechanism-driven stroke neurorehabilitation. The presentation is divided into two key areas. First, we examine the underlying neural mechanisms of Motor Imagery (MI) training for post-stroke limb rehabilitation. We integrate Virtual Reality (VR) environments to enhance patient engagement and motor planning, utilizing Functional Near-Infrared Spectroscopy (fNIRS) to monitor dynamic hemodynamic changes in the motor cortex. The goal is to objectively validate the neural response induced by the VR-MI paradigm, optimizing the therapeutic delivery for functional motor improvement. Second, we focus on the objective evaluation of post-stroke aphasia. We employ Magnetic Resonance Imaging (MRI)-based Functional Connectivity (FC) analysis to map aberrant or preserved language networks . This approach seeks to move beyond subjective, behavioral scales by identifying distinct neurobiological signatures within the brain's connectome, thereby enabling a more precise and neuro-biologically grounded classification system for aphasia subtypes. The ultimate goal of our research is to develop highly personalized, mechanism-driven rehabilitation protocols for stroke survivors.
